The incidence of cancer in adolescents and young adults is increasing, with an average annual increase of 0.3% between 2010 and 2019. The incidence of cancer in adolescents and young adults varies by age and sex. The most common cancers in adolescents and young adults in Europe are those that affect the reproductive system, such as testicular cancer and breast cancer, as well as melanoma, thyroid cancer, and lymphoma2. More specifically, adolescents aged 15-19, the most common types of cancer are testicular cancer, Hodgkin lymphoma, thyroid cancer, and brain and other central nervous system tumors. For young adults aged 20-29, the most common types of cancer are breast cancer, melanoma, thyroid cancer, and testicular cancer. For young adults aged 30-39, the most common types of cancer are breast cancer, melanoma, colon and rectal cancer, and non-Hodgkin lymphoma.
The incidence rate of breast cancer in Europe among adolescents and young adults is approximately 31.8 per 100,000 people per year for females6. The incidence rate of testicular cancer in Europe among adolescents and young adults is approximately 137.4 per million.
The late effects of breast cancer among adolescents and young adults in Europe can include adverse effects from treatment, such as pulmonary, cardiac toxicity, reproductive dysfunction, pneumonitis, arm lymphedema, neuropathy and skin changes late effects910. The late effects of testicular cancer among adolescents and young adults in Europe can include permanent or late-occurring side effects from treatment, such as nerve damage to the fingers or feet, hearing loss, and sensory neuropathy.
Management and ongoing support for testicular and breast cancer survivors in Europe may include: 1) regular follow-up appointments with healthcare professionals to monitor for late effects and manage any symptoms that arise; 2) physical therapy and rehabilitation to address physical late effects, such as lymphedema or axillary web syndrome; 3) cognitive-behavioral therapy or other forms of counseling to address anxiety, depression, or changes in body image; 4) fertility preservation and counseling for testicular cancer survivors, as treatment can impact fertility; 5) education and support for survivors and their families to help them manage the emotional and practical challenges of living with a cancer diagnosis and its late effects. Therefore, it is important for healthcare professionals to work closely with survivors and their families to develop individualized care plans that address their unique needs and concerns.
The INSAYACS proposal (Integrated solutions for adolescent and young adult cancer survivors: enhancing understanding and management of late-effects) will contribute to highlight and support healthcare providers in Europe to address the unique needs of young adults’ survivors of testicular and breast cancer by developing specialized support programs and digital tools to address these needs. These programs will include counselling, personalized support, and other resources to help young adults cope with the challenges of cancer treatment and survivorship. In this way, the project will advance the understanding and management of late-effects in AYA testicular and breast cancer survivors, improve health outcomes, and promote equity in access to care and support services across Europe.
INSAYACS Specific objectives:
SO1: Understanding gaps and needs in the management of late-effects in AYA cancer survivors
SO2: Development and Testing of an AI-DSS and a Wearable to Provide Personalized Recommendations for Late-Effect Management, Treatment Optimization, and Supportive Care Interventions
SO3: Optimize Cancer Treatment Regimens to Mitigate the Risk of Late-Effects, integrating AI-DSS and wearable
SO4: Develop Innovative Holistic Interventions
SO5: Maximization of the Impacts, Clustering and Policy Recommendations - Advantages and Innovations
- Through a multidisciplinary approach, the project will comprehensively understand and address the unique needs and challenges faced by AYA cancer survivors, with a specific focus on late-effects management, treatment optimization, and holistic interventions. By leveraging cutting-edge technologies such as Artificial Intelligence Decision Support Systems (AI-DSS) and wearables, INSAYACS will deliver personalized recommendations for late-effect management and treatment optimization while ensuring equitable access to high-quality care and resources across different regions in Europe. Additionally, the project will develop targeted interventions that integrate education, sports, nutrition, medical follow-up, and counseling to address the physical, emotional, and psychosocial needs of AYA cancer survivors.
